[Xine-vdpau] patch to work around DVB subtitles in HD content

Hi

DVB subtitles (ETS 300 743) are limited to 720x576 and display incorrectly 
when mapped to a HD video with a size larger than that.  The attached patch 
works around this by setting then mis-using the osd extents (width & height) 
for indexed OSD content and forcing an output surface to match the requested 
size - in effect zooming the subtitles to match the video.  It should not affect 
RGBA osd's adversely, and has no effect at all if extents are not set.  This 
seems to work fine in my extremely limited testing, but I have only 1 stream 
that contains dvb subs to test with.
